A new program designed to help patients transition smoothly from hospital to home will be the focus of the next meeting of the area’s Parkinson’s Support Group.
The meeting is set for Tuesday, Aug. 5, from noon to 1:45 p.m. at the Anderson Inn at Quarry Hill in Camden and is open to all those living with Parkinson’s disease as well as their loved ones and caregivers.
Patricia LaVallee, part of Pen Bay Medical Center’s new Care Transitions team, will explain how she and other “coaches” work to give adult Medicare patients and their caregivers the support they need for a safe and comfortable return home after a hospital stay.
Support group meetings take place regularly on the first Tuesday of each month, from noon to 1:45 p.m., at the Anderson Inn. Admission is free. Attendees are generally encouraged to bring bag lunches; Quarry Hill provides beverages and desserts.
