As of November 1, 2011, we will be destroying inactive medical records from 1998 to 2001.  Records of current patients will not be destroyed. 

Please contact our office by mail or by calling (207) 596-7148 if you wish to receive your medical record. 

For deceased patients, relatives must provide documentation of next-of-kin status or similar authorization to receive medical information.
